A 62-year-old woman is evaluated in the intensive care unit for new-onset toe pain. She was admitted 5 days ago for a massive pulmonary embolism and has been receiving a continuous intravenous unfractionated heparin infusion. Her temperature is 37.2°C (99.0°F), blood pressure is 138/86 mm Hg, and pulse is 98/min. Physical examination reveals dark purple discoloration and ischemic changes in the toes of her left foot. Laboratory studies show a platelet count of 88,000/mm³, a significant decrease from her admission platelet count of 250,000/mm³. A presumptive diagnosis of heparin-induced thrombocytopenia (HIT) with arterial thrombosis is made, and anti-platelet factor 4 (PF4) antibody testing is ordered.
Arrange the following pharmacologic management steps in the required chronological sequence according to standard treatment protocols for this condition.
- 1Discontinue the intravenous unfractionated heparin infusion
- 2Initiate an intravenous argatroban infusion
- 3Initiate oral warfarin therapy
- 4Discontinue the intravenous argatroban infusion
